We would like to thank you for taking part in our process of completing athletic participation paperwork online. This automated process will allow us to be more efficient in handling the forms as well as saving valuable resources. All forms must be filled out electronically. No paper copies will be accepted. The Physical Evaluation form is the one exception.

• Please read these instructions before proceeding to the forms.

•You must have your student’s ID number available, as it is required on the forms.

•DO NOT USE ATHLETE NICKNAMES ON FORMS. FULL LEGAL NAME ONLY.

• Please ensure you select the correct school for your athlete for the upcoming school year.

• Please fill out all information as requested. If you are unsure about the answer to a question, please answer N/A, unknown, or none. All fields must have something in the box.

• To sign the document click inside the signature box and hold your mouse down. This will allow you to create an “Electronic Signature”. If you make a mistake and need to start over, click on the refresh icon next to the signature box.

• Parent/Guardian AND Student signatures are required in order for forms to be accepted electronically.

•Click on “I agree” next to the signature line. Enter your email address and then press “Submit”. You will receive a confirmation email.

YEARLY PHYSICALS

1. Valley View ISD requires an annual physical for 7th-12th grade students participating in athletics.

3. Fill out the Medical History and take the 2nd page to the physician at the time of the exam.

4. All Valley View ISD athletes must turn in his/her physical directly to the athletic trainer in person or by email.

Form(s) must be filled out completely and submitted before the student is cleared to participate in any athletic event (practice/game).
